Anthem Blue Cross & Blue Shield

Our Health Care Plan provides comprehensive coverage for full-time employees and their eligible dependents. The cost of the coverage is paid for by Intermarkets and begins the first day of employment. Applications for coverage must be received within 30 days from the date of employment or the employee may enroll during the open enrollment period (April 1-April 30).

Employees may enroll in the Anthem Blue Cross and Blue Shield Keycare Preferred Provider Plan (PPO), a plan that uses the Blue Cross and Blue Shield nationwide network. The plan features office visit copayments, preventive care, and in and out-of-network coverage. Our plan also includes a prescription drug program that offers prescription copayments and a mail order pharmacy.

Dental Care Plan

Our Dental Care Plan covers preventive, basic, and major services for full-time employees and their eligible dependents. The cost of the coverage is paid for by Intermarkets and begins the first day of the month following the date of employment. Applications for coverage must be received within 30 days from the date of employment or the employee may enroll during the open enrollment period (August 1 - August 30).

The Dental Care Plan allows participants to see any dentist they wish. However, there is a network of dentists who offer a reduced fee schedule for services. Out-of-network benefits are paid based on reasonable and customary limits. Preventive care is 100% covered up to the reasonable and customary limit as determined by the carrier. There is an annual deductible applicable to basic and major services and an annual benefit maximum.

Vision Plan

Our Vision Plan provides coverage for full-time employees and their eligible dependents. The cost of the coverage is paid for by Intermarkets and begins the first day of employment.

Employees are automatically enrolled in the Anthem Blue View Vision Plan when they enroll in the company's Anthem Keycare Health Care Plan. Benefits include an eye exam, eyeglass lenses and frames or contacts once every twelve months. The Plan also provides additional discounts toward other incurred expenses.

Life Insurance

Intermarkets offers company-paid term life insurance in the amount of $50,000. Employees may enroll in this plan on the first of the month following their date of hire. Applications for coverage must be received within 30 days from the date of employment or the employee may enroll during the open enrollment period (August 1 - August 30).

Section 125 plan

Our Section 125 Plan allows full-time employees to set aside a portion of their paycheck on a pre-tax basis to pay for out-of-pocket expenses related to health care for themselves and eligible dependents. These contributions can then be applied toward premiums for medical insurance, dental insurance, vision care insurance and out-of-pocket medical expenses or dependent care expenses on a "before tax", rather than an "after tax" basis. Your premium contributions and qualified expenses are deducted from your gross pay before income taxes and Social Security is calculated.

Employees are eligible for the Section 125 plan once they have been with the company for 6 months. The window to enroll is open for 30 days after an employee's 6 month anniversary or the employee may enroll during the open enrollment period (Nov. 1 - Dec. 15).  To enroll or access you Section 125 Plan, click here.
